If I bind over and over/underhand attacks to scroll wheel down I can only use the underhand after my overhead in a combo, would there be any way to make so people can over/underhand without having it bound to LMB as well? To make that simpler, I'd like to have a chivalry control scheme but with directional attacks.

Probs because they don't have the skillsets to do that, so instead they are doing other useful work to save time in the long run, while the people qualified for it work on the server stuff right now

It doesn't take much skill to join a server to see if it works.
